Referrals and self-referral

Dentists across Northern Ireland and Ireland can refer patients for an orthodontic assessment using our dentist referral form. Private patients can self-refer using our self-referral form or by contacting the practice directly.

We still accept paper referrals by email (info@turkdentalcare.co.uk) or post (Belfast Orthodontics, Turk Dental Care, 114 Castlereagh Road, Belfast BT5 5FR). Need more referral pads? Email or call us on 028 9045 1303 and we will post them out. See paper referrals for dentists.
